How do I know if I really need PT?

Physical therapy can benefit a range of individuals, from those recovering from surgeries or injuries to those aiming to improve mobility or reduce pain. If you're experiencing physical discomfort, restricted movement, or seeking preventive care, a consultation with one of our Luna physical therapists can help determine if PT is right for you.

Is physical therapy covered by insurance?

Luna accepts many insurance plans and we handle the benefits verification process for you. With Luna, you always pay the same exact co-pay your insurance plan has for visiting a PT clinic. Do I need a prescription from my doctor?

While a prescription can be helpful, it's not necessary to begin physical therapy. All of the states we serve allow direct access to PT without a doctor's prescription to start care. Once your direct access visit limit is reached, our Concierge team will coordinate with your primary care physician if you need a prescription for more visits.

When and where can I have my appointment?

In California, Direct Access rules allow patients to be seen for physical therapy for up to 12 visits with no prescription. After 12 visits or 45 days (whichever is first), the Luna team will work with your doctor to obtain a prescription if more visits or time is needed.

Medicare rules

Medicare requires a signed plan of care for physical therapy. Your Luna therapist will create a plan of care during your first visit, and the Luna team will work with your doctor to get it approved. You will need a new plan of care established every 90 days or 10 visits whichever is sooner.

Home Health Physical Therapy

Home health physical therapy may be part of your care plan if you are having difficulty moving around your home or have had falls due to pain, illness, injury or surgery. For example, in-home physical therapy may be prescribed for you by your doctor if you qualify for home health and you have the following:

  • Surgery like a knee or hip replacement
  • Recent heart attack
  • Acute-onset medical condition
  • Exacerbation of a chronic medical condition like heart failure , COPD or diabetes
  • Trouble with balance or falls
  • Difficulty walking or moving around
  • Pain after an injury, surgery or illness
  • Functional decline related to a progressive neurological condition such as Parkinson’s, Multiple Sclerosis or ALS
  • Suffered from a stroke and have loss of use of one or more extremities

Our at-home physical therapists and physical therapy assistants help you move with greater ease in the comfort of your home. Your plan of care is based on your needs and your doctor’s orders. Physical therapy for seniors is designed to restore normal mobility or maintain the highest level of functioning possible, so you can do daily tasks and activities you enjoy.

WHAT IS PHYSICAL THERAPY?

Physical therapy is a type of care designed to relieve pain and improve people’s ability to move and function easier.

A doctor may order in-home physical therapy for seniors who have had an injury, surgery or illness that makes it hard to do daily tasks.

If you need home health care physical therapy, you’ll work with a physical therapist – an educated, licensed professional with specialized training. These specialists understand how the body works and what it needs so you can do the things you care about most. Our physical therapists create personalized plans using a variety of treatment techniques such as balance retraining, stretching and strengthening exercises, gait training, teaching fall reduction strategies , physical agent modalities such as ultrasound, electrical stimulation and hot and cold therapy, and patient and caregiver education.

WHERE IS HOME HEALTH PHYSICAL THERAPY PROVIDED?

With home health care physical therapy for the elderly, a physical therapist comes to your home. This can be your private residence, a family member’s home, a group home or a senior living community.

Diane's Story

Diane had several health issues – hypertension, diabetes and heart disease – that were limiting her ability to move around the house and keep up with basic tasks like getting dressed. In home health physical therapy, she worked on balance and strength, which allowed her to walk longer distances and get back to things she loved like church and visiting her family.

Dan had a recent stroke and struggled to get around his house without a walker. Physical therapy helped minimize his need for a walker, so he could stay as active as possible.

A car accident resulted in multiple fractures to Malcolm’s legs and back and a prolonged hospital stay. WHAT TO EXPECT FROM PHYSICAL THERAPY HOME VISITS

With home health care physical therapy, your therapist comes to your home. At your first physical therapy home visit, they will evaluate your needs. You might be asked:

  • Are you having pain or other symptoms? What level is your pain?
  • Do you have difficulty doing daily tasks like getting out of bed, getting dressed or bathing?
  • How are you sleeping?
  • Do you have a medical history of illness or other health problems?
  • What types of things would you like to be able to do?

Your Amedisys physical therapist might ask you to walk, bend, grasp, climb steps or do other motions to better understand your needs. They will also look at your balance, strength, flexibility, range of motion and posture. At this initial physical therapy home visit, you’ll learn about your treatment plan and how often your physical therapist will visit. You’ll also set up a schedule.

At subsequent physical therapy home visits, you’ll discuss how you’re feeling and any changes since your last visit. You’ll practice exercises and do activities to help you reach your personalized goals. Your physical therapist can teach you how to move safely and help identify environmental hazards at home. They can provide education for your caregiver as well.

At the end of each visit, your Amedisys physical therapist will share updates and progress with the rest of your home health team. This way, all team members who visit you will understand how you’re doing.

HOW LONG IS PHYSICAL THERAPY?

The number of home health physical therapy visits you receive depends on your needs and doctor’s orders. Some patients receive physical therapy for six to eight weeks. If you have a serious or progressive condition, you might require more visits. Alternatively, if your condition improves quickly, you may have only a few home health physical therapy visits along with a home exercise program.

How long is each physical therapy visit? A typical visit is 45 minutes to an hour but the length of physical therapy depends on your needs. Some physical therapists see patients once or twice a week initially, and then once a week once they’re stabilized.

HOME HEALTH PHYSICAL THERAPY GOALS

You’ll work with your doctor and home health team to set up a treatment plan and goals. Your specific focus areas will depend on your condition and needs. Some common home health physical therapy goals include:

  • Reduce pain and swelling
  • Improve your ability to move around

  • Prevent disability or further deterioration

MISCONCEPTIONS ABOUT HOME CARE PHYSICAL THERAPY

There are many misconceptions about physical therapy, especially in the home health setting. Here are a few misconceptions as well as the facts:

Some people think physical therapy is just a series of exercises that they can’t realistically maintain on their own. But it’s much more than exercise, especially in the home environment.

In addition to helping people recover from injuries, accidents and surgeries, physical therapists can also help prevent injuries and falls and improve patients’ abilities to do daily tasks. By addressing problems early on, patients can save money and minimize their risk of surgery and hospitalization later.

The goal of physical therapy is to reduce pain, not add to it. Your physical therapist may ask you to move in ways you aren’t used to, but treatment should not hurt. There’s no need to delay care to avoid making pain worse. Your physical therapist should check in with you about your comfort level, so be sure to tell them if something bothers you.

Massage and exercise can be part of physical therapy, but physical therapists can do much more. Physical therapists are movement experts who improve quality of life through prescribed exercise, hands-on care and patient education. They have specialized knowledge and skills and have completed an accredited physical therapy program with a bachelor's, master's or doctoral degree in physical therapy. Physical therapy assistants (PTAs) have completed an associate or bachelor's degree. Both PTs and PTAs are also licensed by the state.

The purpose of at home physical therapy is to empower patients to manage their condition on their own. Physical therapists teach you the tools to take control of your illness or recovery. With home health care physical therapy, you learn to use tools and equipment that are readily available in your own home. If you do your daily PT activities, the benefits can last long after therapy sessions end.

IN-HOME PHYSICAL THERAPY COST

Does Medicare cover physical therapy at home? If you’re eligible for home health and your doctor orders physical therapy as part of your plan of care, the in-home physical therapy cost is typically  covered by Medicare , Medicaid and some private insurance plans.

ARE YOU ELIGIBLE FOR HOME HEALTH PHYSICAL THERAPY?

To be covered by Medicare and Medicaid, you must meet eligibility criteria for home health care . These include:

  • You’re considered homebound. This means it’s very difficult for you to leave home and you need the help of a supportive device (walker, wheelchair, etc.) or another person to do so.
  • You need part-time or intermittent skilled care. This may include physical therapy, skilled nursing, speech therapy or an ongoing need for occupational therapy.
  • Your doctor orders home health for you.

Ask your doctor if home health care is an option for you.

If you don’t meet these home health criteria, you can still get private physical therapy at home if you pay for it out of pocket or through private insurance. Or you may want to consider a local outpatient clinic.

Home-care physical therapy occurs when a physical therapist comes to your home to provide rehabilitation services. This type of service is reserved for patients who are unable to leave the house to attend PT. This can occur for a variety of reasons, including:

  • Your condition is acute or severe and leaving the home presents a health risk
  • Your functional mobility is limited and you cannot travel to therapy
  • You lack adequate transportation or access to transportation to get to your physical therapy
  • You simply desire a private PT session in your home

Most physical therapy services in the United States are covered by insurance, and certain rules apply to home-care physical therapy to be covered by your insurance carrier.

For most insurance plans, physical therapy in your home will be a covered service if you are able to demonstrate significant barriers to you leaving the house. These barriers may be due to your injury or illness or due to your inability to travel.

If you choose to have PT at home just because it is convenient for you, your insurance company may not cover the service and you will have to pay for the therapy yourself.

How Do You Get In-Home PT?

If you suffer an injury or illness that limits your ability to move around normally, you may benefit from PT. And if you cannot leave the house, home-care physical therapy may be the best option.

Most patients who receive home-care physical therapy are referred to the service by their healthcare provider. Your healthcare provider can make the referral to a local visiting nurse service that offers physical therapy, and he or she can attest that home services are medically necessary. This medical necessity report will likely also state that home-care physical therapy is needed because you are unable to leave the house due to your condition or circumstances.

If you feel you may benefit from physical therapy at home, you can also call your local physical therapy clinic or visiting nurse association. Those organizations can help you navigate the pathway to receiving home-care PT.

So how do you know if you can benefit from home-care physical therapy? In general, if you have any problem that results in a loss of functional mobility, you may benefit from working with a physical therapist. And if you are having difficulty leaving the house, a PT can come to your home. Conditions that may require home-care physical therapy may include:

  • Post-surgical conditions like a total hip or total knee replacement
  • Parkinson's disease
  • Multiple sclerosis
  • Amyotrophic lateral sclerosis (ALS)
  • After a cardiac event like a heart attack
  • After trauma such as fracture

If you have a problem that keeps you from moving normally and if it is burdensome to leave your home for therapy treatment, you may benefit from having your rehab in your home.

What If I Don't Qualify?

Sometimes, patients want to have home-care physical therapy, but their insurance company denies coverage. This may happen if your insurance company determines that your condition does not warrant in-home physical therapy. It may also occur if you have not proven that you are unable to leave your home or if leaving your home for PT presents a significant burden.

If home-care services are denied, you may have a right to appeal. Call your insurance company and ask about an appeals process. You may also wish to call your healthcare provider's office to explain your situation. Your healthcare provider may be able to help you navigate the appeals process.

If you do not qualify for home-care PT, you can attend physical therapy at a local outpatient clinic. Of course, you will have to travel to the clinic for treatment. Your outpatient physical therapist should be able to provide you with home exercises to do in order to limit your trips to their facility for rehab.

You may also wish to simply pay a private home-care physical therapist to come to your house for therapy if you do not qualify for services. However, this option may be financially burdensome.

Is In-Home PT Effective?

Many people wonder if home-care physical therapy is effective. Why? Because many believe that you need expensive, heavy equipment and exercise tools to engage in an effective rehab program. This is not necessarily true. Your home-care physical therapist is trained to utilize available resources to maximize your rehab experience. He or she can also bring light equipment to your home to ensure you get the most out of your physical therapy.

A 2016 study published in the journal Orthopaedic Nursing examined the functional outcome of patients after total knee replacement surgery.   Some of the patients in the study had home-care physical therapy followed by outpatient PT, while other patients attended outpatient PT only. The authors of the study found no difference between the two groups when looking at function and range of motion (ROM) measurements. Having PT at home after knee replacement did not provide significant benefit, but it also did not contribute to a negative outcome.

Another small study found that patients who suffered upper extremity hemiparesis and weakness after a stroke had an improved outcome and satisfaction with outcome after receiving in-home therapy services. This study only included six patients, so it is not considered very powerful. Still, it indicates that there may be a positive benefit to home-care physical therapy.

While having home physical therapy, you will hopefully get better. As your condition improves, your functional mobility may improve, and you may be able to start leaving the home regularly. If this happens, your home-care physical therapist may discontinue in-home services. At that time, you may continue your rehab in an outpatient PT facility.

A Word From Verywell

Working with a physical therapist after illness, injury, or surgery can help you recover full functional mobility. This can allow you to return to your normal work and recreational activities. Sometimes, your injury or your personal circumstances may prevent you from safely leaving your home to attend physical therapy. If this is the case, you may benefit from home-care physical therapy. Working hard at home with your PT can help you have a positive outcome to regain maximal functional mobility.

Vestibular Physical Therapy

Some insurance plans cover out-of-network physical therapy visits. Connect with your insurance company ask about your plan’s coverage.

  • Some clients have out-of-network physical therapy benefits and may receive partial reimbursement.
  • Some clients have in-network benefits, but have a high deductible that must be met before physical therapy is covered.
  • Some clients do not have physical therapy benefits and pay per visit regardless of if the clinic is in-network or out-of-network.

If you have out-of-network physical therapy benefits, you will be provided with a “superbill” for you to submit to your insurance. Call your insurance and ask:

  • Are you required to receive pre-authorization prior to your initial visit/evaluation?
  • Do you need a physician referral, primary care referral, or a specialist referral?
  • What is your deductible? How much has been met? What percentage of reimbursement does your plan allows for?
  • Does the percentage change with a non-preferred provider?
  • Is there a cap to the amount of visits or money spent annually?
  • How does your plan ask you to submit? Is there a specific form they provide? See this example from Blue Cross of MA .
  • Verify the correct mailing address or fax number for claims submission.

Each patient will experience a different treatment program designed especially for them. By working with the patient and their loved ones physical and/or occupational therapists can care for patients to provide the following therapies:

When a patient is referred to PT or OT, their first visit will be an evaluation. This means the therapist will ask about the patient’s physical history, examine the area of the body that needs therapy, and make a recommendation for treatment. Patients will be asked to come in once a week or more frequently, depending on their care plan.

Inpatient Therapy

Physical and occupational therapists work with patients while they are in the hospital to help them regain independence and function. They can also help in making sure the patient goes to a safe setting when they are ready to leave the hospital. This could be home, but could also be to a non-hospital setting that helps with physical therapy

PTs may evaluate range of motion, strength, coordination, balance and cognition as they relate to functional mobility, including bed mobility, transfers, ambulation, wheelchair mobility and stairs. 

OTs may evaluate range of motion, strength, coordination, balance, and cognition as they relate to activities of daily living (ADLs) including bathing, dressing, toileting, and self-feeding.

At Mobile Physician Services, our customized care teams provide patients with both comfort and familiarity as they work with a dedicated primary care provider and care coordinator to improve their health. Our physicians and staff take a proactive approach to preventive care, chronic disease management, and chronic illness support right where you live.

Each home visit includes an in-depth examination and individualized treatment plan, which is monitored and adjusted through routine follow-up visits. The primary care provider will deliver your ongoing care and will recommend to you specialty services as needed.

  • Annual Wellness visits : This wellness visit allows your primary care provider to create or update your personalized prevention plan. This visit includes a review of your medical and social history related to your health and may include counseling about preventive services. This plan may help you to prevent or reduce the chances of future illness based on your current health and risk factors.
  • New Illness Exams : When a new symptom or ailment arises, call us. Early indications of not feeling well could be a clue that you may be getting sick. A symptom in one part of the body may also be a sign of a problem in another part of the body. Moreover, unrelated symptoms that might seem minor on their own, could be warning signs of a more serious medical disease or condition. The new illness exam can be very brief or more detailed depending on your concerns and the provider’s findings.
  • Follow-up Care: Involves a regular medical checkup, which may include a physical exam and laboratory testing. Follow-up care checks are a proactive way of assessing the potential for and preventing health problems from returning after treatment of a disease has ended or an illness has seemingly passed.
  • Referral for Specialty Care : Referrals are the link between primary and specialty care. The referral coordination includes the documentation of patient care activities, the transfer of information, the inter-provider communication itself, and the integration of care services to the patient. Mobile Physician Services is a multi-specialty practice so many of these specialty referral services can be made seamlessly with little inconvenience or disruption to the patient and caregivers regular routines.
  • Medication Management: Medication management is a treatment structure that ensures our patients are receiving optimal therapeutic results from their prescription medications, both in the short and long term. Our team’s goal is to mitigate medication noncompliance and monitor all prescriptions treatments so that drug interactions complement one another for the most optimal outcome for our patients.

  • Depression : Depression is a common and often serious medical illness that negatively affects how you may feel, the way you may think, and how you may act. Depression can cause feelings of sadness, despair and hopelessness, which may lead to a loss of interest in activities you once enjoyed.

Our wound care specialists have been trained in the attention and treatment of all types of acute and chronic wounds. They have skill and experience in wound debridement and wound care procedures – managing chronic, non-healing wounds and infections, with a demonstrated care that fosters healing… right in the patient’s own home.

We specialize in serving homebound patients who may also be bed-bound or have difficulty in walking or moving around. As a result, immobility compression sores develop at pressure points on the body when the weight of an immobilized individual rests continuously on a firm surface, such as a mattress or wheel chair. Often these same patients are on oxygen or have high-risk medical conditions which makes it an even more challenging and stressful effort for them to travel to a doctor’s office for an appointment. Thus, the necessity for in home care and treatment.

During a visit, your doctor will exam each eye for signs of serious issues such as glaucoma, cataracts, macular degeneration, and detached retinas, among other conditions.

Receiving regular eye exams regardless of the state of your vision can help detect serious eye problems at their earliest stages ─ when they are most treatable. During an eye exam, your doctor will observe and evaluate the health and condition of the blood vessels in your retina, which can be good indicators of the health of your blood vessels throughout the rest of your body.

  • Comprehensive eye exams : This exam goes beyond a simple vision screening. A comprehensive eye exam includes a host of tests in order to do a complete evaluation of the health of your eyes and your vision.
  • Annual retina exams : A retinal exam allows your doctor to evaluate the back of your eye, including the retina, the optic disk and the underlying layer of blood vessels that supply the retina.

  • Diabetic eye exams : Diabetes does not have to lead to vision loss. Taking an active role in managing your diabetes can go a long way in curbing later complications. Regular eye exams, good management of your blood sugar and blood pressure, along with early intervention for vision problems can help prevent vision loss caused by diabetic retinopathy. Retinopathy is caused by damage to the blood vessels of the light-sensitive tissue at the back of your eye.
  • Refractions: This test is given as part of your routine eye examination. It is often referred to as a vision test. This test assists your eye doctor in measuring you for the exact lens prescription you will need.
  • Eye injuries: Eye trauma could be the result of a sudden blow to the eye. It may cause the eye to suddenly compress and retract which could cause damage to your eye and the surrounding tissue. Even if the injury may seem minor, all eye injuries should be furthered examined by a doctor for possible more serious and underlying injuries.
  • Eye infections: The most common eye infection is conjunctivitis, also known as pink-eye. An eye infection can happen in almost any part of your eye, including your eye lid, cornea and optic nerve. Symptoms of eye infections may include redness, itching, swelling, discharge, pain, or problems with vision.
